Neuroimaging often investigate alternative diagnoses in older comorbid patients. By using MR imaging, we assessed “occult” brain white matter (WM), brain gray matter (GM), and cervical cord damage in patients with neuroborreliosis in an attempt to achieve a more accurate picture of tissue damage in these patients, which might contribute to the diagnostic work-up. Most patients with Lyme neuroborreliosis have neuroimaging without pathology. • MRI enhancement of meninges or nerves is consistent with Lyme neuroborreliosis. • More than half of patients with Lyme neuroborreliosis have neuroimaging performed. • Neuroimaging often investigate alternative diagnoses in older comorbid patients. In neuroborreliosis, magnetic resonance imaging (MRI) may show meningeal and cranial nerve enhancement after contrast administration [3–5] and/or signal abnormalities in the white matter of the brain, although rarely reported .

Late Lyme neuroborreliosis, defined as continuous disease lasting more than 6 months, represents less than 2% of all Lyme neuroborreliosis. Definite diagnosis can be made in the presence of suggestive neurologic symptoms, CSF pleocytosis, and intrathecal Bb antibody production.

After three weeks of treatment with 100 mg/day doxycycline and 2 g/day ceftriaxone sodium, his symptoms and CSF abnormalities were rapidly improved. 2006-04-01 The MRI revealed a strong contrast enhancement of the conus medullaris and fibers of the cauda equina, while the 18 F-FDG PET/CT was without pathological findings.

Lyme neuroborreliosis (LNB) is neurologic involvement secondary to systemic infection by the spirochete Borrelia burgdorferi in the United States and by Borrelia garinii or Borrelia afzelii species in Europe.
